Financial Assistance

Notre Dame High School uses FACTS Grant & Aid Assessment to determine the extent of financial aid given to families. We ask Families applying for tuition assistance to complete the assessment as soon as possible at: www.factstuitionaid.com If you prefer to complete the assessment by mail, you can pick up a form in our school.

 Applications and all supporting documentation must be mailed directly to Facts Grant and Aid Assessment. Please use the FACTS Grant & Aid Assessment self-addressed envelope to mail the following information:

 Signed and completed paper application (not applicable if applying online);

• A non-refundable $20 application fee;
• Copies of your most recent tax forms including all supporting tax schedules;
• Copies of your most recent W-2 form for both you and your spouse; and
• Copies of supporting documentation for Social Security Income, Welfare, Child Support, Food Stamps, Workers’ Compensation, and TANF

The school cannot provide financial aid unless the FACTS assessment has been completed
